My CPU sports a similar behavior and I'd guess it is because the tempdiode is close to the die and is heavily affected by small changes in powerusage. What are the chances that on screensaver it's running hot due to graphics, but the temp doesn't update until it wakes from slumber? I do not think so...

So, what your are saying is just use the Fan mate on lowest speed (by the way where do you mount the FanMate inside the case?), and not use Cool ´n´Quiet, because does not work with Zalman?! I assume that if a Nexus gives you 16-19dBA it means the minimum noise at power supply idle state with maximum 60W DC power load. This 30dBA maybe means maximum noise at full load. If you read for exampel the review on nexus 4090 you'll see that the best it will do noicewise is 21dbA at those loads...
