1. Driver for AMD
2. Windows Power Managment = Minimal

Four things are required for Cool'n'Quiet...
  1. You need to download and install the AMD Athlon™ 64 Processor Driver for Windows XP and Windows Server 2003 Version (exe) 1.2.2.2
  2. You need to enable Cool'n'Quiet in your BIOS
  3. You need to enable ACPI 2.0 support in your BIOS (if this option is available). Some motherboards have built-in support for APCI 2.0 and offer no option to control it in their BIOS.
  4. You need to set the power management to "Minimum power management" in Windows.
After that, Cool'n'Quiet should work properly. A tool like CPU-Z is helpful to monitor your CPU speed and voltage, and see if Cool'n'Quiet is really working.

Why don't you use the Crystal CPUID aplication, it has a review somewhere on silentpcreview.com? With it you can control Vcore, multiplier, and the time before and after the new frequency to be modified. IT's quite good, but i noticed problems with Prime95 wich reports errors when Crystal CPUID has enabled Multiplier Management.

That sounds to me like you have set up the Multipliers wrong - volts being set too low and you are experiencing instability...
