PSU OVP (over voltage protection)

Corsair RM1000i died. Two main switchers dead, their drivers and the drivers' driver - the resonant controller ICE2HS01G. The PFC part and the fuse survived. The graphic card's power circuit killed, obviously, by more than 12V.
OVP did not do what it was supposed to do. Your opinions, please. Bad design? Does it happen? Would it happen again, if repaired?

It's hard to determine what's the original problem here.
If the resonant controller died first it's possible the switches got into an "always on" state, resulting in a sudden very high output voltage.
The OVP may then not have had time, or even been unable to, react. (Unable if it relies on the resonant controller to cut the power.)

To me it seems like you just got a bad sample, and a repair should fix the problem.
