The card comes with a Thermalright HR-03GT heatsink (a $50 value !). Add your choice of 92mm fan (fan clips included) and you're good to go. I had a 92mm Nexus @6V on it and it did wonders.
I also put heatsinks on all the memory modules and on the power circuitry. The PCI plate was modded to allow more air to come in from the back (see pics).
