This is just a guess...

Is it because it's a PWM fan? IIRC, PWM fans run at full voltage (12v). So they may not be as flexible as most fans are in terms of starting voltage. Maybe you should use a non-PWM fan.

The purpose of putting a fan in that position is to supply the cards with cool fresh air, not to push the hot air out the back - why not try it and see if it helps? Ok, so I put a fan there. With both cards in, case closed, the cards idled at 48C. Definately an improvement from when I started this ...

The only issue I see with running a 120mm fan there is the pressure. One fan surely can't push the hot air the cards create out the back....

Yes, you do. SATA drives need 2 cables. The thin data cable, and a power cable, which looks like this. The black end goes into the drive, the white end plugs into a Molex connector. That image is an adapter btw.
